By signing below, each of above of above listed all business and business owner/officer (individual collectively, "Applicant") certifies that Applicant is and authorized to sign on behalf of the above named company, all information and supporting documentation submitted with this application is true and accurate, and such information may relied upon evaluation this application, Applicant authorizes Orem Financial Services LLC dba Exit Your Business ("EYB") to share documentation with any of its representatives, all this application and all supporting successors, designees, and third party lenders ("Assignees"). Applicant authorizes EYB and Assignees to request, receive and review any consumer of personal, business and Investigative reports and other information about Applicant, including credit card processor statements and bank statements, from one or consumer reporting agencies, such as TransUnion, Experian and Equifax, and from other credit bureaus, banks, creditors and other third parties for the purpose of evaluating eligibilities for commercial credit. Applicant also consents to the release, by any creditor or financial institution, of any information relating to any of Applicant to EYB and to each of the Assignees, on its’ own behalf. Furthermore, you hereby waive and release any claims against EYB, all Assignees, and any information-providers arising from any act or omission relating to the requesting, receiving or release of the information obtained in connection with this application.
